How do paparazzi's take pictures in crowds? (photography question)?

When they hold their dslr up high in an angle to try to get pictures of a person in a crowd do they use auto focus or do they pre focus? What kind of lens do they usually use for that? Because AF can take a while and you're in a crowd too. MF can be unreliable.
